Search Form?

i have database name "prj"contains one table name "inf"which contain three fields "id" , "name" , "domicile".I want to create a php search form which contain the dropdown menu of fields name. that i can search the record by id by name by domicleand as a result show the {id, name , domicile } as record infromation. How it can be possible, please help
