Where to add an colspan?

I have been trying to get my form aligned but I can't get the last field in place. I have tried all kind of colspan's but it won't change. Can someone tell em where to add the correct colspan?

presumming you have 5 column table, and want label and input in one cell, horizontal inline (not llabel above)
