PHP form with Muse CC.

I am trying to figure out a way to create a form in my Muse CC site. I have done some form tuts with W3School but the elements of the form are compact and close together. I am trying to understand how I might be able to spread the form elements out into existing web pages matching the location of art work that would represent choices say for Radio Buttons.

 

I am also not able to locate an example of creating multi page php form that would collect data from multiple pages then write that to either a txt, csv or send through email.

 

 

Any ideas would be great.
