How to create text and radiobox combination

I want to creating donation form that combine radiobox and textbox in one name.

Yes, it's the same form.

as you can see, user can choose 10,25,50,100,250 and can do manual input via form input type "text".

when user choose 25, form input type "text" filled 25

I think the poster is looking to attach a onclick event to each button with specific value to run a function to retrieve that value and update/add text input value to that clicked button value.
