Template view VS MVC

So far in the app i am building, i am enclosing in functions the output code of the webpage, and then, these functions are "included" in index.php or other page for that matter. Then, I am going to use a different page(other the one used for output) for DB connectivity Is what i describe above the template view?If yes, is it easy at all to convert it to MVC? What kind of code goes into the controller file and what in a model file? Am I correct to assume that DB code goes into the model file? What goes in the controller file-give me a practical example because in theory I know what a controller does.
