Insert data inline of preset texts

Dear all,

 

Before I begin, I would like to firstly introduce myself. I am Joel Kuan from Malaysia. Hello!!

 

Anyways, I have a question in regards to creating a form.

 

I have created a basic form but I would like the text inserted by the user to be placed inline a preset text.

 

for example:

 

The form:Name:_______Age:_________

 

Final result:

 

My name is {name}. I am {age} this year.

 

I tried searching on google but somehow I can't seem to get the right keyword to find this. Sorry!
